Adenocarcinoma in situ (AIS) of the cervix is a potentially precancerous lesion with increasing incidence. Consequently, physicians frequently perform colposcopically directed biopsy of cervical dysplasia to more reliably estimate the presence of AIS, to gauge its clinical behavior, and to elucidate the influence of treatments on the risk of preinvasive or invasive carcinoma. From 1988 to 1995, 29,993 women underwent colposcopically directed cervical biopsy. Among them, 12 had a pretreatment diagnosis of cervical dysplasia and were included in the study if the patient agreed to undergo a histologically directed biopsy at the biopsy site. Formalin-fixed, paraffin-embedded material was stained with hematoxylin and eosin. Four patients were found to have cervical intraepithelial neoplasia 3 (CIN 3) and eight patients were found to have AIS. Three patients had a lesion that was less than 3 mm in diameter. The presence of a lesion at the biopsy site was associated with white cell (P =.0004) and epithelial cell (P =.0017) infiltration of the stroma. Lesions associated with stromal infiltration were larger in size (P =.003), and were more likely to contain atypical cells (P =.0006). Treatment with acetic acid (P =.03) and LEEP (P =.02) were predictive of the presence of tumor. AIS of the cervix appears to be a variant of cervical intraepithelial neoplasia 3 with greater depth of stromal invasion. Lesions associated with stromal infiltration are larger in size and are more likely to contain atypical cells.
