Recently, several studies suggested that a single cigarette smoking could adversely affect the circadian blood pressure (BP) rhythms of hypertensive patients. However, there is no clear evidence to support this suggestion. The objective of the present study was to evaluate the impact of cigarette smoking on circadian BP rhythms in a large number of subjects with mild or moderate hypertension. Ambulatory BP measurements were performed in 161 patients with mild (n=95) or moderate (n=66) hypertension. Ambulatory blood pressure recordings were obtained over 24 hr by using a Spacelabs 90207 monitor. All patients were questioned in detail regarding lifestyle habits. The numbers of patients with or without a history of cigarette smoking, defined as habitual smokers, were 33 (21%) and 58 (39%), respectively. The 24-hr mean BP and BP loads were significantly higher and lower, respectively, in habitual smokers than in non-habitual smokers. In both groups of hypertensive patients, smoking status affected the night/day ratio in DBP only. Of all patients, the casual BP measurements were significantly higher in habitual smokers, and this difference was most obvious in the daytime. The casual BP measurements, however, did not differ significantly between two groups according to the smoking status. In conclusion, this study demonstrated that cigarette smoking significantly impacts
